Making a Referral

How to Make a Referral

The referral must be in writing- dated and signed by the student’s parent or legal guardian. The referral should:

Describe the concerns about your child’s development, learning and/or behavior
State that you are requesting a special education evaluation (be sure to write "preschool special education evaluation" if your child is in preschool)
List any services your child has received or is currently receiving
Provide your child's full legal name and date of birth
Provide your name, address and a telephone number where the Child Study Team can reach you
State your preferred language, if it is not English

Where to Send the Referral

If your child is currently a Winslow Township Student, provide your written letter to the principle or staff member at your child’s school.
If your child is not a Winslow Township student, mail your original letter to:

What to Expect after the Initial Referral Has Been Made

Initial Identification and Evaluation Meeting

Within 20 calendar days (excluding school holidays, but not summer vacation) of receipt of written request by the district, the CST must convene with the parent for an Initial Identification and Evaluation Meeting, to determine whether an evaluation is warranted.

You will receive an invitation to meet with the CST to determine whether an evaluation is warranted.
If determined at the meeting that evaluations are warranted, you will receive a “Consent to Evaluate” your child, in the mail.
Please sign and return this consent to the Special Services Office or your child’s school.

*If no consent is signed and returned within 15 days after the consent was sent home, this referral process is terminated and a new Referral is required to reinstate the process.

If Evaluations are Not Warranted

Written notice will be sent to your home address via US Mail, which proposes that evaluation is not warranted. The notice will also include copies of the special education rules (N.J.A.C. 6A:14), and the due process hearing rules (N.J.A.C. 1:6A).
